By checking you allow us permission to photograph your child.

By checking you allow us to use your child's photo for church promotion.

By checking you aknowledge that in the event of a medical emergency, someone from the Western Avenue Baptist Church will contact you at the phone number you provided. The person/persons in charge at WABC have your permission to seek medical attention for you child at the nearest medical facility to their location, at the time of the emergency. You understand you are responsible for all medical expenses concerning your child.*
